Checkbox doesn't show up

In this form there's a checkbox list at the end. The checkboxes don't show up.
I bought a licence of laytheme and the ninja forms as suggested on your website.Can you help?

they are still there. Have a look here.
I think that LayTheme overwrites some CSS settings from Ninja Forms. With CUSTOM CSS you can probably style them like you want.

Before you post:
- When using a WordPress Cache plugin, disable it or clear your cache.
- Update Lay Theme and all Lay Theme Addons
- Disable all Plugins
- Go to Lay Options → Custom CSS & HTML, click "Turn Off All Custom Code", click "Save Changes"
