Form (created with forminator) is not showing up.

Hej People,
I created a form for a clients website.
As in the documentation suggested I used the plugin Forminator.In the past I used Happy Forms but it seems not to work anymore on Laytheme. (If I submit the form it just shows JSON Code and the information is not send.)
When I embed the form via Shortcode it just doesn’t show up. If I am logged in, I’ll get a link “Formular bearbeiten” (Edit Form) – if I am in private mode / logged off nothing shows up. It seams not to load at all.

I can find the form in the inspector and the element shows
display: none;in html source code it shows
data-forminator-render="0"Ajax is aktivated, i tried to deactivate it (and in the form as well) – didn’t help. Caches are cleaned, tried to Enable Content Filter. Deactivated and switched cookie banners.
I enabled Smart Loading for HTML and Shortcode Elements, as I need this for some check out form.
Has anybody an idea / any suggestions?

Before you post:
- When using a WordPress Cache plugin, disable it or clear your cache.
- Update Lay Theme and all Lay Theme Addons
- Disable all Plugins
- Go to Lay Options → Custom CSS & HTML, click "Turn Off All Custom Code", click "Save Changes"
